Patient Engagement Group

Dear Patient,

We would like to know how we can improve our service to you and how you perceive our surgery and staff members.

To help us with this, we are setting up a patient engagement group so that you can have your say on services provided by the GP practice. We will ask members of this representative group some questions from time to time and discuss certain areas and concerns which need to be raised.

We aim to gather around approximately 15 patients from diverse backgrounds which will allows us to gain a truly representative sample.

If you are willing to join our PEG (patient engagement group), please attend the Alice Street surgery and ask for a registration form at the reception. Alternatively, you can open the link below, download and print the form at your own convenience and return it to the practice once filled out. Please note that medical queries will not be responded to.

The practice continues to provide efficient services to our patients based on their needs and demands. In house surveys allow us to improve our services and all other areas of concern raised by patients and service users.

Once we receive feedback from the surveys, we will discuss results of surveys with members of our active Patient Engagement Group.

Our Patient Engagement Group’s Chair is Mr Shazad Hussain and vice chair is Mr Fazlul Haq.
