GPOS National GP SURVEY, 418 GPOS surveys were sent out to our patients 56 responded to this and 13% completed the survey; based on the poor uptake we decided to do an in-house patient survey please see attached results. We will soon be publishing an action plan after the PPG forum has discussed this.
Assisting people who live in the Bradford District and who have a long-term illness which may be made worse by cold living conditions.

We want to improve our service to you – and we need your guidance to ensure this happens.
The Patients’ Fourm is being developed so that you can tell us what you think.
The practice is looking to further develop its Patient Participation Group and will welcome patient representatives from the practice. They would be a strong advocate for patient choice and voice. The practice would welcome feedback in how we can improve services and access to the practice.

The practice PPG chair is Shazad Hussain. The practice has a dedicated noticeboard in the waiting room.
The practice has appointed Rizwana Shaheen as the practice public engagement lead. She will worlk with the Practice Engagement Group. community groups and the practice to develop patient voice and engagment in order to improve practice services.
